FIREARMS REGISTRATION 

The Andover Township Police Department processes all firearms applications for Andover Township residents. For more information, please contact Judy McSherry in the Records Department at (973) 300-0462 or jmcsherry@atpd.org.

Items Needed for Firearms Application

• Completed Application
• Completed Mental Health form - Part One (this form needs to be signed in the presence of a police department personnel)
• Completed "Employment Verification" form (Questions 1-5 only), signed & dated
• Copy of birth certificate or passport
• Copy of current firearms ID Card
• Certified Check or Money Order in the amount of $60.25 made payable to "Division of State Police/SBI" (new applicants only)
• If you already have an SBI number or a firearms ID card, you will need to complete a "Request for Criminal History Record Information" form (SBI 212A) and submit it along with a Certified Check or Money Order in the amount of $18.00 made payable to "Division of State Police/SBI"                                                                                        

  Note: Form SBI 212A (Request for Criminal History Record Information) must be submitted to the State Police using the original yellow form. For this reason, we are unable to make it available for download. Please pickup the form at Police Headquarters.

Note: Do not submit two separate payments. Only one payment is required - either $60.25 for new applicants or $18.00 for applicants who already have an SBI number.
